Indonesian Health Minister to Run for WHO Director-General

Indonesia’s Health Minister Budi Gunadi Sadikin has officially become the fourth candidate to run for the Director-General of the World Health Organization. The nomination was listed on the WHO election website over the weekend, adding a non-medical professional to a field of contenders that has traditionally been dominated by medical experts.

A Non-Medical Profile in Global Health

Sadikin brings a unique background to the race. By training, he is a nuclear physicist and a former banker who served as the Chief Executive Officer of Indonesia’s largest bank, Bank Mandiri, starting in 2013. Before entering government service, he was also the Group CEO of the state-owned mining and aluminium enterprise PT Inalum until 2019.

The minister took over the Indonesian Ministry of Health in December 2020, leading the country through the COVID-19 pandemic without any formal medical or public health background. During an event on pandemic preparedness on the sidelines of the World Health Assembly, he remarked that the previous president placed him in charge “to take care of COVID.”

Observers note that all previous Director-Generals – with the sole exception of Dr. Tedros Adhanom Ghebreyesus – were qualified medical doctors. Tedros holds an MSc in infectious diseases from the University of London, whereas Sadikin’s experience lies in corporate leadership and finance.

Financial Expertise and Diplomatic Credentials

Despite the lack of a medical degree, Sadikin’s strong background in finance could be a decisive advantage in the current climate. As a key diplomatic credential, he is also one of the architects of the World Bank-hosted Pandemic Fund, launched in Bali in 2022.

Indonesia contributed some $30 million to the WHO last month to help close the outstanding funding gap. Dr. Tedros Adhanom Ghebreyesus posted a flattering thanks to Sadikin on LinkedIn after the pledge, though the WHO DG has not openly endorsed any candidate for the upcoming race.

The agency faces significant financial strain. As of May, the budget for the 2026-27 base programme remained short of $300 million in funding – even after the originally proposed budget was pared back by over $1.1 billion since the United States withdrew from the global health agency in January 2025.

Sadikin’s nomination brings the current field to four declared contenders ahead of the 24 September deadline. The declared candidates are informally regarded as the current “frontrunners” in the election, scheduled for May 2027 in Geneva.

While this financial and diplomatic background offers a distinct profile, the agency faces a time of tumult and change. Funding shortfalls triggered by the United States’ withdrawal have forced far-reaching cost-cutting measures, reducing staff by 25% over the past year. And over the past month, several members of the senior leadership team have left the organization – including the leading UK physician and researcher Dr. Jeremy Farrar, whose departure was confirmed only last week. Farrar had been considered a potential DG candidate but has denied that he is running.

Member States will elect the new leader during the 80th World Health Assembly.
